Ol' Painless is a Small Guns weapon in Fallout 3. It's a unique variation of the Hunting Rifle with a higher damage rate and 0 spread, meaning any shots taken outside of V.A.T.S. mode will move perfectly in the direction you aimed.
Ol' Painless can be found in Dave's safe in the Republic of Dave, the safe is unpickable and can only be opened by killing or pickpocketing Dave or by rigging the Republic of Dave election so that Dave doesn't win during an unmarked quest called Election Day, once either Bob or Rosie win the safe will open.
